Personal injury reports
built for litigation
Documentation designed to strengthen demand packages, support mediations, and withstand IME scrutiny. Custom narrative reports with 48-hour turnaround from a QME-certified chiropractor.
Our Reports
Documentation that drives settlement value
Every report produced at adjust.clinic is written with the understanding that it may be challenged by opposing counsel, reviewed by an independent medical examiner, or presented to a jury. That awareness shapes every sentence.
Our reports are not template-driven. Each one is a custom narrative that tells the story of the injury, connects the mechanism to the diagnosis, and establishes causation using language and methodology that hold up under cross-examination. We write for the three audiences that matter most: the insurance adjuster evaluating your demand, the mediator assessing case value, and the defense expert looking for weaknesses.
Dr. Ryan Todd Lloyd is a Qualified Medical Evaluator appointed by the California Division of Workers' Compensation. That designation means his clinical opinions are held to a higher standard than those of a typical treating chiropractor -- and his reports reflect that rigor. Every finding is documented with specific grades and measurements, every opinion is supported by clinical reasoning, and every conclusion is tied to evidence.
Reports are produced from our Petaluma office at 3100 Lakeville Highway, conveniently located just off Highway 101 and serving personal injury attorneys throughout Sonoma and Marin counties.
Report Components
What every report includes
Six core components that transform clinical findings into litigation-ready documentation. Each element is designed to support a specific aspect of your demand.
Mechanism of Injury Analysis
Detailed analysis connecting the collision dynamics -- direction of force, vehicle damage, occupant position -- to the patient's specific injuries. We correlate impact forces with known injury thresholds from peer-reviewed biomechanical literature, establishing the physical plausibility of each diagnosis.
Clinical Findings
Comprehensive documentation of orthopedic testing results, neurological examination findings, range of motion measurements, palpation findings, and functional assessments. Every finding is recorded with specific grades, measurements, and clinical significance -- not vague generalizations that defense experts can dismiss.
Diagnostic Imaging Correlation
Integration of X-ray, MRI, and CT findings with clinical presentation. We identify pre-existing degenerative changes versus trauma-related findings, explain the clinical significance of each imaging result, and demonstrate how diagnostic imaging supports -- or modifies -- the working diagnosis.
Causation Opinion
A clear, reasoned medical opinion on whether the injuries are causally related to the accident, written to the standard of reasonable medical probability. Each opinion is structured to withstand Daubert and Kelly challenges with proper methodology, reasoning, and citation of supporting clinical evidence.
Treatment Plan & Medical Necessity
Evidence-based treatment recommendations with medical necessity justification for every modality, frequency, and duration prescribed. Each recommendation is supported by clinical reasoning and outcome data, ensuring that no element of the treatment plan can be credibly challenged as excessive or unnecessary.
Impairment Ratings & Future Care
When appropriate, permanent impairment ratings per AMA Guides, 5th Edition -- the standard required in California. Future care recommendations include treatment types, frequencies, durations, and cost projections using local market rates, providing the data your demand calculations require.
When You Need Us
Report types for every stage of your case
From initial evaluation through final disposition, each report type serves a specific purpose in building and supporting your client's claim.
Initial Evaluation Report
Comprehensive baseline documentation following the first examination. Establishes the patient's pre-accident condition, initial injury presentation, mechanism of injury analysis, working diagnoses, and a detailed treatment plan. This report supports early case valuation and sets the foundation for all subsequent documentation.
Progress Reports
Periodic documentation of treatment response, functional improvements, and clinical milestones generated at regular intervals throughout the course of care. These reports demonstrate the patient's trajectory toward maximum medical improvement and create the narrative of recovery that strengthens your demand over time.
Discharge & Final Report
Comprehensive summary of the entire treatment course including total visits, treatment outcomes, final diagnoses, and -- when applicable -- permanent impairment ratings per AMA Guides 5th Edition. This is the capstone document that ties together every element of the clinical record into a cohesive narrative for your demand package.
Narrative Medical-Legal Report
A detailed standalone report designed specifically for demand packages, mediations, and trial preparation. Includes a full case summary, mechanism of injury analysis, causation opinions, treatment history, outcome analysis, and future care recommendations with cost projections -- all in a single comprehensive document.
Supplemental Reports
Additional reports responding to IME findings, new diagnostic imaging results, changes in clinical status, or specific questions from the handling attorney. When the defense produces an IME report that challenges your client's injuries or treatment, we provide a detailed, point-by-point clinical response that addresses each objection with evidence and reasoning. These supplemental reports ensure that no defense argument goes unanswered in your demand package or at trial.
